Whether your patient needs physiotherapy, exercise physiology, occupational therapy or a combination of services, we provide timely appointments, thorough assessments, and regular progress updates back to you — so you stay informed at every step.

Referral Process

How to Refer a Patient

Our referral process is designed to be quick and straightforward. Most patients are contacted within 1–2 business days and seen within a week of referral.

Send Your Referral

Email your referral to Admin@nexushealthgroup.au or call 0484 592 762. Include the patient’s details, diagnosis, relevant medical history, funding type, and the service you’re referring for. We accept referrals in any format — a letter, completed referral form, or even a brief email with the key details.

We Contact the Patient

Our team reaches out to the patient within 1–2 business days to introduce ourselves, confirm their funding details, discuss their needs, and schedule their initial appointment at a time that works for them.

Assessment, Treatment & Feedback

We conduct a comprehensive assessment, develop a personalised treatment plan, and provide regular progress reports back to you as the referring professional. You’ll receive an initial assessment summary and ongoing updates so you can stay informed about your patient’s care.

What to Include in a Referral

Patient Details

Full name, date of birth, contact number and address.

Clinical Information

Diagnosis, relevant medical history, precautions, current medications, and any imaging or specialist reports.

Funding Type

NDIS (plan-managed/self-managed), DVA (Gold or White card), Medicare CDM plan, Support at Home, or Private.

Service Requested

Physiotherapy, exercise physiology, occupational therapy, or general allied health assessment. If unsure, we’re happy to discuss the best option for your patient.
