Support at Home

The Support at Home program helps older Australians access allied health services to maintain their independence, safety, and quality of life at home. Our occupational therapists provide in-home therapy across the Perth metropolitan area, while our physiotherapy and exercise physiology services are delivered in clinic. All three disciplines accept Support at Home funding — working alongside your home care provider, GP, and family to deliver coordinated, practical support.

Staying Independent at Home

As we age, maintaining mobility, strength, and daily function becomes increasingly important — but it doesn’t have to mean leaving the comfort of your own home. Our clinicians work with older Australians and their families to deliver therapy that supports long-term health, reduces fall risk, and helps you continue doing the things you enjoy.

We coordinate closely with home care providers, GPs, and other health professionals to ensure your therapy aligns with your broader care plan and goals.

How We Support You at Home

Our occupational therapists come to you — providing hands-on care in the environment where it matters most. Physiotherapy and exercise physiology services are provided in our clinic. Here are some of the ways we can help.

Mobility & Strength

Tailored exercise programs designed to maintain and improve your ability to move safely around your home and community. Our physios and EPs work with you to build strength, improve walking confidence, and keep you active for longer.

Falls Prevention

Falls are one of the leading causes of hospitalisation for older Australians. We provide balance and coordination programs, strength training, and environmental risk assessments to significantly reduce your fall risk and help you feel more confident on your feet.

Home Safety Assessments

Our occupational therapists assess your home environment to identify hazards and recommend practical modifications — from grab rails and non-slip surfaces to bathroom renovations and improved lighting. We provide detailed reports that can be used to apply for funding through your home care package.

Equipment & Assistive Technology

The right equipment can make a huge difference to daily independence. Our OTs assess your needs and recommend aids — such as walking frames, shower chairs, bed rails, and kitchen adaptations — that make everyday tasks safer and easier. We can also help with funding applications and equipment trials.

How It Works

Accessing our services through Support at Home is straightforward. Your home care provider, GP, or family member can arrange a referral — or you can contact us directly.

Your home care provider, GP, or family member contacts us on your behalf — or you can reach out directly. We’ll confirm your eligibility and discuss what services would be most beneficial.

In-Home Assessment

One of our occupational therapists visits you at home to assess your needs, review your living environment, understand your goals, and develop a personalised therapy plan. This visit also helps us identify any immediate safety concerns or equipment needs.

Ongoing Therapy & Support

Regular occupational therapy sessions in your home to improve your health, safety, and independence. Physiotherapy and exercise physiology sessions are held in our clinic. We provide progress updates to your home care provider and GP, and adjust your plan as your needs evolve over time.

Support at Home FAQs

Who is eligible for Support at Home?

Support at Home is available to older Australians (typically 65+) who have been assessed and approved for a home care package or Commonwealth Home Support Programme (CHSP) funding through My Aged Care. If you’re not sure whether you’re eligible, your GP or home care provider can help you apply.

Do you come to my home?

Our occupational therapy services are delivered in your home across the Perth metropolitan area. Home-based OT allows us to assess your actual living environment, recommend modifications, and identify safety risks specific to your home. Our physiotherapy and exercise physiology services are provided in our clinic but accept the Support at Home funding scheme.

Can family members be involved?

We strongly encourage family and carer involvement. Having family members present during sessions helps with continuity of care, allows us to teach carers how to support exercises between visits, and ensures everyone is aligned on goals and progress. We’re also happy to communicate directly with family members if that’s preferred.

How do I arrange a referral?

Your home care provider can arrange a referral on your behalf — this is the most common pathway. Alternatively, your GP or a family member can contact us directly. We’ll handle the coordination with your provider to ensure funding is approved and services are set up smoothly.
